Apply For Assistance

While we understand that you would like to get through this process quickly, it's not simply a matter of exchanging emails and sending a check to your veterinarian. We must evaluate all applications carefully.

 

Before filling out your application, please review application process and guidelines.

 

There are three ways to apply for assistance:

  1.  Fill out our Online Application
  2.  Email a completed version of our Application to triage@labradorlifeline.org
  3.  Fax a completed version of our Application to (978) 912-5053

Ensure that you print out a hard copy for mailing, also. It is recommended that you retain a copy for your records, and for ready reference when discussing the application with your case sponsor.

 

One very important note: E-mail is our primary means of communication. If you have given us your email address on your application, please check your email DAILY (including any SPAM or FILTER Folders) to see if we have responded to you. Failure to respond to our inquiries means a delay in processing your application. 

 

There may be a few day’s lag time between the date you apply and the date of our initial response, but we will get back to you as quickly as we can (please remember that we are all volunteers, and we sometimes get a lot of applications so most of our members work on more then a single application at a time).
