You may choose ONE of the following options for submission. The options available to you are:
Faxing or e-mailing an application, or submitting it via our web site on-line, will allow the Labrador Life Line Triage Committee to assign a case sponsor and begin evaluation of your application; the regular mail address is provided in case you have no access to email however please note that regular mail routing will add time to the process.

Ensure that you print out a hard copy for mailing, also. It is recommended that you retain a copy for your records, and for ready reference when discussing the application with your case sponsor.
One very important note: E-mail is our primary means of communication. If you have given us your email address on your application, please check your email DAILY (including any SPAM or FILTER Folders) to see if we have responded to you. There may be a few day’s lag time between the date you apply and the date of our initial response, but we will get back to you as quickly as we can (please remember that we are all volunteers, and we sometimes get a lot of applications so most of our members work on more then a single application at a time). Thank you for your help in this matter.
